Referral Process

Our Intake Specialist are available to pre-screen referrals, to consult with physicians and other professional referral providers to discuss the needs and supply information about our program to enable you to decide if our treatment option is the best fit for your client.

Our Intake Specialist will strive to make the referral process as easy as possible. We value all of our referral sources and understand how concerned you are with your clients’ well-being.  It is important to us not only to provide superlative care for every child or adolescent referred but also to maintain regular communication with you, so you are always up to date on how your client is progressing.

We also know that you are far more familiar with your clients’ current status than we are when they arrive and will take into account any information you feel it is important for us to be aware of in order to better meet their specific needs.  It is our intention to work collaboratively with you not only during the evaluation and treatment stages but continuously through discharge. 

It is our commitment to help everyone who comes through our doors regain a sense of hope regarding what the future will bring and instill a sense of confidence that they can achieve the goals to establish the life they dream of living.

Referral packets can be sent directly [email protected] for an Admissions Specialist to review.

Referral requests will not be reviewed without the following (as applicable):​
  • Current insurance plan information
  • Annual Assessment (most recent)
  • Current treatment plan listing all applicable (diagnosis codes)​
  • Current medication list and most recently changed medications
  • Current Individualized Service Plan​/Service Plan signed by BHP​​
  • Psychiatric progress notes (if currently admitted into a Level 1 facility)​
  • CFT notes covering a 60 day period​
  • IEP (if applicable)​
  • Completed Brief Intervention Plan (BIP) (if applicable)​
  • Diversion requirement records (if applicable)​
  • An in-person interview may be required​
